P3.F119: Organ Complications in Pediatric Patients with Severe Dengue at the Hospital General Plaza de la Salud (HGPS), Dominican Republic, During the period 2016-2019

Background: A Dengue infection can evolve to Severe Dengue affecting multiple organs such as muscle, liver, kidney, heart and brain during its course, aggravating the prognosis of the patients. The damage to these organs contributes to patient mortality, which in combination with an increase in Dengue incidence, has caused an alarm in tropical and sub-tropical countries while also posing a risk for serologically immature travelers worldwide. To this day, a Severe Dengue infection in endemic countries is considered one of the principal causes of hospitalization and death in pediatric patients. This epidemiological study aims to present the different organ complications that can be present in pediatric patients diagnosed with Severe Dengue and hospitalized in Hospital General Plaza de la Salud during the period 2016-2019, focusing on the prevalence and frequency of these appearances. Our objective is to contribute to the statistical data and motivate a more specialized attention to the different manifestations, which can be a significant obstacle during the treatment and management of this viral disease.

Methods: This was a retrospective cross-sectional study where data was collected from inpatient database with a confirmed Dengue diagnosis through IgM plasma detection during the period 2016-2019. The study analyzed 277 pediatric patients hospitalized with Dengue with warning signs and severe Dengue.

Results: Out of 277 pediatric patients with Dengue with warning signs, 30 were diagnosed with severe Dengue, were 28 presented organ complications. The most commonly affected organ was the gallbladder with 6 cases, followed by myositis presenting 5 cases. The least affected organ was the pancreas with 2 cases. (Table 1)

Conclusion: Severe complication of organs was observed in 73% of the cases of pediatric patients diagnosed with severe Dengue between 2016-2019 (Table 2). The most common signs and symptoms of the patients diagnosed with Dengue with warning signs and Severe Dengue were gastrointestinal. The most affected age range was between 3-12, conforming 77% out of the 30 patients with severe Dengue. 13.60% of patients diagnosed with Severe Dengue with organ compromise suffered a comorbidity, which was most commonly asthma.
