P3.A001: 5210 Implementation through community partnerships

Purpose/Objectives: This project aims at endorsing healthy “5-2-1-0” habits to all the children aged 2-9 years old encountered during well child visits and thus prevent obesity in early ages.
· Evaluate baseline habit choices using AAP-Healthy Habits questionnaire
· Make clinics 5-2-1-0 friendly
· Engage local youth leaders and develop a multimedia method for 5-2-1-0 message
. Identify most effective educational means to provide counseling

Design/Methods: PDSA Cycle I will be to determine local data trends at our clinic and the proportion of 2 year old pediatric patients visiting the clinic for well child visits who received education regarding the “5-2-1-0” mnemonic which is a policy opportunity tool described by AAP for promoting healthy living in children and preventing obesity. This data collection will involve chart review of the patients attending “UF Health North clinic”. The keywords used to screen the charts will be “5-2-1-0”,” counseling regarding eating 5 servings of fruits and vegetables”, “counseling regarding less than 2 hours of screen time”, “counseling regarding 1 hour of physical activity”, “counseling regarding zero sugary drinks”
-Further PDSA Cycle will build on the baseline data generated in the first phase to advocate for developing and implementing strategies to promote usage of the preventive strategy “5-2-1-0” for every patient attending the clinic.
1. Via partnership with “I am a star foundation” which is led by high school students, we will develop multimedia message(video) for promotion of 5-2-1-0 healthy habits. The children will be able to identify with the youth conveying message. We will show this message to the children and parents during their well child visits. This intervention will be followed by assessment of impact via “Healthy habit questionnaire” post the intervention.

Results: We will have a documented baseline assessment of “Healthy habit questionnaire” pre intervention and will assess the improvement in the practices of healthy habits at home 1 month and 3 months after intervention. These set of results (pre and post intervention) will be compared after each PDSA cycle. The results are being analyzed and shall be ready to be presented by the time of presentation

Conclusion/Discussion: It is a multi-setting clinic-based intervention with community partnership to address the most burning health issue of today’s time. The AAP recommends counseling parents at every well-child check on lifestyle goals as a part of obesity prevention initiatives and “Let’s go 5-2-1-0”was originally used by the Maine Center for Public Health's Maine Youth Overweight Collaborative. This QI project , with help of CATCH grant from SOAPM aimed at inculcating these healthy habits' counseling with well child visits through innovative methods.
